Welcome to becominglocal.co, and thank you for choosing PT Jelajah Budaya Lokal for your cultural journey through Bali’s rural communities.

Around 7-14 days prior to the tour, customer with valid booking will be prompted to fill the Personal data and consent/waiver form.

To ensure a safe, respectful, and well-prepared experience, each group representative is required to complete the Personal Data and Consent/Waiver Form. This document serves the following purposes:

To ensure we understand your health, dietary, and emergency needs
To acknowledge and inform you of any associated risks
To comply with the Personal Data Protection (PDP) Law of the Republic of Indonesia
To allow our team to prepare appropriately for your tour

All personal information collected is confidential, handled in accordance with applicable regulations, and deleted within seven (7) days following the conclusion of your tour.

Form Overview

The form is divided into three main sections:

Section 1: Personal Data

This section gathers essential information about the group and its primary contact person, including:

Booking number, full name, phone number, and email address
Country of passport/identification and last 3 digits of passport or ID
Accommodation address in Bali (to coordinate meeting points)
Personal eligibility declaration, confirming participants are at least 19 years old and do not require assisted mobility devices

This information helps our team verify your booking, coordinate logistics, and ensure the tour is suitable for all participants.

Section 2: Health Information

This section collects relevant medical and dietary information for all participants:

Declaration of any chronic medical conditions
Dietary preferences (e.g., vegetarian, halal, pescetarian)
Food allergies (e.g., peanuts, dairy, shellfish, soy, gluten)

This information is kept confidential and is used solely to make necessary adjustments to meals and activities for your safety and comfort. Health data is securely deleted within seven days after your tour concludes.

Participants arriving via international flights are also reminded to complete the Indonesian government’s health declaration form, available at sshp.kemkes.go.id.

This section consists of formal acknowledgements and consents:

Personal Data Protection Law Compliance

In accordance with UU RI No. 27 Tahun 2000, the form requests your consent for the following:

To collect and use your personal information (e.g., name, email address, WhatsApp or Telegram contact)
To contact you regarding your inquiry, booking, or changes
To forward your information to third parties when necessary (e.g., for accommodations or transportation)
To allow you to withdraw consent at any time by contacting connect@becominglocal.co, upon which your data will be removed from our system within three working days

Acknowledgement of Risk

By signing the form, you acknowledge that participation in the tours involves potential risks, including:

Travel-related accidents
Exposure to allergens or local foods
Physical activity in rural settings, such as uneven terrain or traditional tools
Interaction with natural elements or animals

You understand that while PT Jelajah Budaya Lokal takes all reasonable safety precautions, some risk cannot be eliminated.

Waiver of Liability

You agree to assume all known and unknown risks voluntarily and release PT Jelajah Budaya Lokal, its staff, guides, volunteers, and partners from any liability or claims arising from your participation, unless caused by proven negligence.

You also confirm that you, and any minors under your care, are mentally and physically capable of joining the activity, and that all submitted information is accurate and complete.
